I have tried to debug this problem, and at least in my case, it seems that $sfgFormPrinter is not found to be set in SM_FormInputs.php even though Semantic Forms is already loaded. Is it possible to provide a link to your wiki, so I can determine what's going wrong? Are you sure everything got uploaded correctly? There should be an SM_FormInput.php in SemanticMaps/FormInputs/.
